There are several codes that may apply to pipe markers in industrial workplaces. The ANSI A13.1 code is the one that applies in most circumstances. There is also ASME A13.1, which is the exact same code issued by a different organization. ANSI and ASME work together to produce a consistent standard. Unmarked Electrical Panels Contribute To OSHA Citation

OSHA has cited Fernandes Construction of Glendale for 10 alleged serious violations of workplace safety standards at a construction site located at 167 Blackstone St. in Woonsocket after an employee sustained fatal injuries from falling 50 feet through a skylight.

"One step in the wrong location can have devastating and irreparable consequences," said Patrick Griffin, OSHA's Rhode Island area director. "Falls are the leading killer in construction work, but they can be prevented with effective guarding and training. I urge all contractors in Rhode Island to examine their fall protection programs, make any necessary improvements, and ensure that their workers are properly trained to recognize and avoid fall hazards on the job."

An inspection conducted by OSHA's Providence Area Office found that the skylight opening lacked a guardrail or equivalent means to prevent workers from stepping on or through the skylight. In addition, open holes on walking/working surfaces at the site were not covered, and employees were not trained to recognize fall hazards.

Inspectors also identified violations involving a hand-fed circular saw that lacked proper guarding to protect employees, electrical components that were not covered to prevent employees from possible contact, unmarked electrical panels and the lack of ground fault circuit interrupters. A serious violation occurs when there is substantial probability that death or serious physical harm could result from a hazard about which the employer knew or should have known.

ANSI/ASME A13.1 Pipe Marking Standards

According to the most recent ANSI/ASME A13.1 update, it is recommended that all piping systems be labeled with the direction of flow (using arrows at one or both ends of the label), pipe contents, and potential hazards. The pipe markers need to be visible from any angle the piping system is approached, near valves, and at every 25 – 50 feet. Although the ASME/ANSI standard suggests labeling all pipes, it is required for pipes to be labeled if the pipe contents could affect procedures during emergency situations. Arc Flash Precautions

The impact of an arc flash fire in Portland, Oregon could have been minimized.

An arc flash is a rapid, explosive discharge of electrical energy that usually results from a short circuit fault.

A small explosion in an underground electrical vault burned a Portland General Electric (PGE) worker recently. Firefighters were called to the location, power went off and evacuations followed. An electrical worker was working on a commercial meter when an arc flash occurred, burning her and starting the fire.

Though further information about the woman and her condition haven’t been released and fire investigators are determining what went wrong, there are general safety precautions that should be taken in areas where arc flashes are likely to occur.

The potential for an injury can be reduced using various techniques such as remote breaker racking, remote door openers and closers, personal protection equipment (PPE) and arc flash warning signs and labels.
